Before Your Visit
What to bring to your first visit
A few simple things help us check you in quickly and make the most of your time with your provider.
- A photo ID A driver license, state ID, or passport.
- Your insurance card Bring the card for the plan you would like to use.
- A list of your current medications Include prescriptions, vitamins, and anything you take regularly, with doses if you have them.
- Any relevant records Recent test results or notes from another provider, if you have them on hand.
New patient forms. There is nothing you need to print ahead of time. When we schedule you, we will confirm exactly what to complete, so there are no surprises at the front desk.
